- test_skills_tool: the 59da8ec4e cross-dir refusal case becomes a parametrized
precedence test (local>external incl. same relative path, create_dir>external,
local>create_dir, project>local) asserting the winner loads, skills_list shows
one row, and the shadowed copy is warned about. Same-tier refusal tests are
unchanged except the equal-rank case also pins the new "use one of" paths.
- test_skill_commands: one end-to-end test that list, prompt index, slash,
skill_view, -s preload and cron agree on a same-tier and a cross-tier duplicate.
- test_org_skill_namespace: a personal/org clash now lists the org copy under its
exact loadable path and notes the unloadable root-level personal copy instead
of flagging two rows that both carried the refused bare name.
