- drop _run_stream_subscribers; the sweep reads _RunStream.subscribers
- per-write timeout via asyncio.timeout (no Task per token), force_close kept
- _sse_frame(id=) instead of hand-prepended id: lines
- reconnect queue gets headroom for the replay length
