unsupportedpastels 9c2d40d60a fix(gateway): preserve managed values and bridge authored subdicts ...

Restore managed extra values after cross-shape merges and expose authored subdict conflicts to plugin hooks before global env fallbacks. Consolidate the precedence coverage into two invariant matrices and qualify documentation for managed policy and explicit env precedence.
