dkbjornn c68d21cda0 fix(kanban): goal gates fail open when the judge transport fails ...

judge_goal returns verdict="continue" with transport_failed=True when the
auxiliary call itself fails (relay 400, auth, timeout). Both terminal handoff
gates — hermes_cli/kanban.py::_goal_mode_handoff_rejection (hermes kanban
complete / request-review) and tools/kanban_tools.py::_goal_gate
(kanban_complete / kanban_request_review) — discarded that flag and surfaced
the infra failure as a judge rejection, so a recorded human approval could
never be applied while the judge was unreachable.

Read the fifth tuple element at both seams and allow the handoff (logged as a
warning) when it is set; a genuine continue/blocked verdict still rejects.

Ported from PR #78605 onto the current gate helpers (the original targeted
the pre-refactor inlined gates in _cmd_complete / _handle_complete).
