Teknium a9e2716c9b fix(codex): confirm a pending call whose call_id first appears on its done item ...

When the announcement carries no call_id and the done item does, the
call_id-exclusive lookup found nothing and left the announcement pending;
response.completed then settled it as a second call. Fall back to the
item/index match only when the matched announcement has no call_id of its
own, so a conflicting call_id still never evicts an unrelated call.
