xxxigm c5a68f0777 fix(fallback): lift a reopened Codex quota window before gating the primary restore ...

A Codex 429 benches the pool entry and arms the session cooldown until a resets_at that can be days out. The pool's throttled usage probe notices an early reopen (redeemed reset, top-up), but only on select(); a session pinned to its fallback gates on the cooldown and next_available_at, which never probe, so it stayed on the fallback for the whole declared window.
