CoolStar 08ac080441 fix(mcp): stdio servers default to the session working directory ...

A stdio MCP server without an explicit cwd inherits the Hermes process's
launch directory, so hosted sessions (ACP, gateway) that pin a logical
session cwd get relative-path servers resolving against the wrong tree.

Default the stdio child cwd to agent.runtime_cwd.resolve_context_cwd()
(session pin, then TERMINAL_CWD), keeping explicit per-server config first
and native None when no anchor exists — hermes's own runtime_cwd single
source of truth.

Adds regression tests (session default, explicit override).
