The unavailable-provider warning was deduped by provider name process-wide,
so a multiplexed gateway or Desktop backend told only the first profile.
It is now keyed by (home, provider).

A first-time install refused at the consent gate said "Reinstall declined
... The installed plugin and active environment are unchanged" although
nothing was installed; migration notices now show that text to users.
