finn763 4a916f7a83 fix(serve): reap orphaned host owner instead of looping observe-only
A prior backend whose session died (dead spawner, PID re-parented to
init) stays alive, so it keeps holding the host flock and its record
passes every staleness check. A fresh backend hit HELD_BY_OTHER and
returned observe-only forever; every attach retried against the same
orphan and its 10s probe timed out indefinitely.

On HELD_BY_OTHER, look the conflicting owner up in the spawn ledger
and apply the established spawner_is_dead discipline (previously wired
only into the mcp-helper reap): a provably dead-spawned owner — or a
null-spawner owner reparented to init, old past the lock-write grace,
and claimed by no Desktop SSH lock — is terminated (SIGTERM, 2s,
SIGKILL), its record discarded, and the host lock re-claimed.
Live-spawned, lock-claimed, young, interactive, self, and unprovable
owners are never touched.
