The ready-lane guards (recent_success, active_pr) stop duplicate
implementation; the review lane is exempt and request-review accepts a
ready card, so a reviewer/closer profile can be put on a card whose PR is
open without DB edits or comment stripping. Operators could see the hold
reason (#112244) but not the route. Part of #111910.
