Teknium 6cd2817a6d fix(plugins): a user-installed model provider reports enabled, as the loader treats it ...

gate_manifest loads kind: model-provider plugins from any source without a
plugins.enabled entry, but _plugin_status only granted that default to
bundled plugins. A catalog-installed provider (claude-subscription-directsdk)
was live yet showed 'not enabled' in hermes plugins list and as a switched-off
row in the Desktop Plugins hub.
