kshitijk4poor 3ffd44e4d3 fix(agent): an init-time fallback carries its own identity into requested_provider ...

The init-time fallback in _routed_client_kwargs switched agent.provider to the
fallback entry but left requested_provider naming the primary. The
answer_in_reasoning gate now scopes trust by requested_provider, so a primary
that fell back at construction to a same-base_url sibling inherited the
primary's opt-in. The runtime fallback, /model switch and MoA paths already set
both fields; this one now does too.
