teknium1 9f7928ecde fix: pin the iteration-limit summary's router-shim rejection through handle_max_iterations ...

The _summary_text consumer of is_router_timeout_shim was wired but no test reached it:
disabling that check left the suite green. Swap one redundant validate_response control
(embedded phrase, already covered by the stream divergence control) for a test that drives
agent._handle_max_iterations: first summary response is the HTTP-200 shim with zero
completion tokens, the second is real text -> the real text is returned after 2 calls.

Also stop re-joining the whole accumulated content on every text delta once the first
delta has been released: a shim cannot follow already-emitted text.
