Teknium 41236dd450 fix(code-health): replay samples a closed merge window and writes a manifest ...

The replay picked PRs with sort:updated-desc, so the sample changed whenever an old PR got a
comment and two runs could not be compared (unsupportedpastels M6). It now takes every PR merged
inside a closed date window in PR-number order, writes manifest.json (number, base, head), and
--manifest replays exactly those ranges. The summary prints one total: PRs with at least one
blocking finding, instead of mixing blocking and warning-only rows.
