Subject: [PATCH] catalog(custom-dangerous-patterns): raise requires_hermes to
 >=0.21.4

0.5.0 gates block patterns through the pre_tool_call approve action
(0.18.1+) and imports tools.approval_detection (0.21.4+); on older
cores block patterns silently stop gating.
